Eldritch Revival (5e Spell)

1st-level Necromancy (ritual)
Casting time: 1 minute
Range: Touch
Components: Verbal, Somatic, Material (100 gp worth of materials, which the spell consumes)
Duration: 1 hour

You may use this spell to attempt to revive a fallen humanoid creature that has been dead for no longer than 7 days. The creature returns to life with 1 hit point. This spell can’t return to life a creature that has died of old age, nor can it restore any missing body parts. Upon reaching the end of the duration, the creature dies once again.

However, the forbbiden nature of this spell is not because it is malevovlent, but it is old, unreliable and prone to misfortune. When the spell is cast, make a skill check using your spellcasting ability modifier. You do not add your proficiency bonus to this check. Depending on the result of this check, the spell has an effect on the ressurected creature as seen on the table below:

Critical FailureThe creature's soul is torn asunder by the spell. The spell fails, and the creature cannot be returned to life by anything short of a wish spell.
4 or lowerThe creature is revived, but the wrong soul is returned to the creature's body. The creature behaves differently than the intended soul, and has different memories and goals than its original host. A random background, personality trait, bond, ideal and flaw is chosen by the DM. Upon reaching the end of the duration of the spell, the creature must make a Wisdom saving throw against your spell save DC. On a fail, the creature dies and the soul is returned back to the dead. On a success, the creature stays in the host creature's body and cannot be removed until the creature is killed.
9 or lowerThe creature's connection to its body is weaker than normal. The duration of this spell is reduced to 1 minute.
14 or lowerThe creature is revived, but they return weakened by the experience. A random ability score is reduced by 1d6 permanently, even if they are revived again. To determine which ability score, roll 1d6, with Strength being 1, Dexterity being 2, Constitution being 3, etc. This reduction can only be undone by a greater restoration spell.
19 or lowerThe creature is revived, but their constitution is faltering after their first death. Their maximum hit points are reduced by 1d12 until they are revived again. This spell cannot reduce a creature's maximum hit points below 1 hit point.
20 or higherThe creature is revived with one hit point. No other adverse effects occur.
Critical SuccessThe creature is revived with one hit point. Upon reaching the end of the duration, the creature is reduced to 0 hit points but does not die.
